Medication Therapy Management (MTM) is a free program offered by Part D plans to certain members to help improve their medication use so they can better manage their chronic conditions (e.g., diabetes, hypertension, asthma) and improve their overall health. MTM is designed to be patient-centric.

What is the purpose of Medication Therapy Management MTM?

Medication therapy management (MTM) is a distinct service or group of services provided by health care providers, including pharmacists, to ensure the best therapeutic outcomes for patients.

You Qualify for MTM if you:

Take eight or more chronic medications & Have three or more of these chronic conditions:

  1. Congestive heart failure (CHF)

  2. Diabetes

  3. End-stage renal disease (ESRD)

  4. Depression

  5. Chronic obstructive pulmonary disease (COPD

You’ll meet with a WCRx Pharmacist who will review all the medicines you’re taking to make sure they’re working the best they can. The goal of MTM services is to make sure your mix of medicines is safe, effective and matches your lifestyle.
